mine comes on during motorway runs, 200cell cat - and i've a o2sim - its always the same code - cat ineficiency - doesn't seem to do any harm
seems to have cleared up since i put the supercharger injectors in

you wont be able to use much of the milltek cat section at all.
basically you need 2 flexi joints iirc 100mmx50mm that fit to the weaponR flange which is cut off from the downpipe section
then 2 x 2" 90 bends , then the collector section also cut off the weapon R downpipe - then to a 90 bend...
its 2.25" i've fitted one, as to the results i'm not sure - i think you also need the turbo U Bend
there was some pics of the internal differences somewhere - but my reckoning was the same as yours.
i'm still considering modding a standard backbox to 2.5" pipework but its a lot of work
astra g 2.0 turbo - you have to remove the ABS ring tho - not too hard of a job

still doesn't answer why the 2.4Tb worked on a bog standard vectra with 2 resistors dunno..
